• Program Planning Data Form, including intention to apply for Prior Learning Assessment and Credit (PLAC)
• Personal Direct Deposit Form

• Final Transcript Submission (if applicable – refer to offer of admission letter for academic conditions/requirements)

In order to complete clinical placements, you are required to upload the following documents to the VERIFIED platform by July 29, 2024:

• Police Vulnerable Sector Check (VSC)
• NOSM University Learner Immunization Form
• Dietitians of Canada Membership & Professional Liability Insurance
• Food Safety Certification
• Research Ethics Course (CORE 2022)-TCPS-2

Your Access to VERIFIED: Log-in credentials to set up your VERIFIED account will be sent to your NOSM University email from Synergy Help Desk in early to mid-July.

Electronic Virtual Review (EVR) with VERIFIED
You must schedule an EVR with VERIFIED to review all of your uploaded documents. The deadline to complete your EVR with VERIFIED is July 29, 2024.

EVRs book up quickly closer to deadline dates, so please ensure you schedule an EVR as soon as possible. EVRs cannot be completed until all required documents have been uploaded to the VERIFIED platform.

NOTE: After the initial EVR, follow-up or missed EVR are $10.00.
