Description: This session aims to provide an understanding of unique considerations in the palliative approach to care for people living with and dying from end stage heart disease.
Learning Objectives:
At the end of this session participants will be able to:
- Identify and gain confidence in the primary level management of the most common symptoms and comorbidities among patients with end stage heart disease including prescribing, de-prescribing, communication tips and prognostication.
- Recognize when and how to access our secondary level experts (Cardiologist and Palliative Care Specialists) in supporting our end stage heart disease patients.
